When: April 6, 2020
Where: Columbus High School Cafeteria
Time: 5:00 pm to 7:00 pm
Required Documents for Registration:
• Government Issued Photo ID (of
person enrolling child)
• Child’s Certified Birth Certificate
(official birth certificate) or
Passport
• Child’s Social Security Card (not
just the number)
• Proof of Physical address
(electric/cable/water bill or voter
registration card showing current
address.)
• Current Immunization Records

You may also register at the Central Administration Office from 8:00 am to 3:30 pm beginning April 7th, 2020. If you have any questions, please call the Administration Office at 979-732-5704.
