Before school starts, make sure your child’s immunizations are up to date. To provide a convenient option for your child’s immunization update, FISD is hosting a Back to School Immunization Clinic on Monday, August 10, 2020, at the FMS Cafeteria. You can view the Texas immunization requirements for enrollment here:

To allow for proper social distancing, we ask that only one parent/guardian accompany their child(ren). Please be bring a face covering and be prepared to wear it when social distancing is not possible. If possible, please follow the schedule below by last name:
A – G 8:30 a.m. – 10:30 a.m.
H – P 10:30 a.m. – 1:00 p.m.
Q – Z 1:00 p.m. - 3:00 p.m.
Open Time / Any letter 3:00 p.m. – 4:30 p.m.

The cost of immunizations are as follows:
Currently Insured – No out-of-pocket expense (A copy of private insurance, Medicaid or Chips must be submitted)
No medical insurance - $5 per immunization
To take advantage of the Immunization Clinic, complete the DSHS Immunization Registry Form and the Aurora Concepts Consent Form attached to this email. Take the completed forms with you to the immunization event. If you have medical insurance, also take your insurance card.
